I understand that participating in the Peninsula Humane Society & SPCA’s (PHS/SPCA) Mutt Strutt, on August 15, 2009 includes an element of risk for my dog and myself. I should not participate unless my dog and I are physically able. I agree to abide by decisions of event officials relevant to my dog’s and my ability to complete this event safely and I agree that event officials may authorize necessary emergency treatment for me and/or my dog. I also understand that vehicle traffic and spectators may be present along the course and I assume the risk of participating under such conditions. I assume all other risks associated with participating in this event, including temperature extremes and the path’s surface conditions. I also affirm that the dog I will be participating with is fully vaccinated. I agree to abide by all event rules. Having read this waiver, I hereby for myself, my heirs, executors, administrators or anyone else who might claim on my behalf, covenant not to sue, and waive, release and discharge the PHS/SPCA, San Mateo County, the City of San Mateo, event officials, volunteers and sponsors, suppliers, employees and other personnel in any way assisting or connected with this event from any claims or liability of any kind arising out of my participating in this event, even though the liability may arise out of negligence or carelessness on anyone’s part. I am more than 18 years of age or my parent or legal guardian has signed the acknowledgment on my behalf. I grant the PHS/SPCA or any sponsor use, for publicity or promotional purpose, my name or pictures of me and/or my dog without liability or obligation to me. You may copy this blank form, but signatures must not be copied. Entries from minors must be accompanied with a parent’s/guardian’s signature. Unsigned registration forms will not be accepted!
